Is there any case law where the failure of the respondent to fully disclose to the court was not deliberate?

British Columbia, Canada

The following excerpt is from Lee (Re), 2021 LSBC 31 (CanLII):

In Law Society of BC v. Antle, 2005 LSBC 45, the hearing panel found that the failure of the respondent to fully disclose to the court was not a deliberate attempt to mislead. As the hearing panel explained at para. 4: It is accepted that he honestly intended to discharge his duty. While he did not intend to mislead, he did mislead, and his conduct amounts to professional misconduct. See also Nejat, at para. 40.
