The following excerpt is from Fritsch v. Magee, 2012 ONSC 2755 (CanLII):

If the parties cannot agree on costs, they may each make written submissions no greater than five pages in length, excluding any offers to settle and the bill of costs. The five pages shall be double spaced, in 12 point font, with proper margins and paragraphs. The plaintiff shall deliver by May 15th and the defendant by May 21st.
